Health Requirements

  • Illinois School Code requires students to submit a report of physical, dental, and vision examinations and the dates that certain immunizations were received in order to attend school.

     

    Ages of Interest/Circumstances Include:

    • Preschool/Early Childhood
    • Kindergarten
    • First Entry in Illinois School
    • Grade Two
    • Grade Six

2020-2021 Health Requirements for Admittance to School

  • A student’s parent(s)/guardian(s) shall present proof that the student received a health examination with proof of the immunizations against, and screenings for, preventable communicable diseases, as required by the Department of Public Health. Unless an exemption or extension applies, the required health forms must be presented to the district by October 15 of the current school year in order for a student to attend school.

     

    PRESCHOOL/EARLY CHILDHOOD

    • Physical Exam
      • Dated within one year prior to the first day of the school year
      • Documentation of diabetes screening
      • Documentation of lead screening
      • Health history portion of physical examination form must be completed and signed by a parent/guardian
    • Immunizations
      • Diphtheria, pertussis, and tetanus (4 doses at proper intervals)
      • Polio (3 doses at proper intervals)
      • Measles, Mumps, Rubella (MMR) (1 dose at proper age)
      • Hepatitis B (3 doses at proper intervals)
      • Varicella (Chicken Pox) (1 dose on or after the first birthday, physician’s statement verifying disease history, or laboratory evidence of varicella immunity)
      • Pneumococcal Conjugate (children 24-59 months of age who have not received the primary series of pneumococcal conjugate vaccine, according to the recommended vaccination schedule, shall show proof of receiving one dose of pneumococcal vaccine)

     

    KINDERGARTEN OR FIRST ENTRY IN ILLINOIS SCHOOL

    • Physical Exam
      • Dated within one year prior to the first day of the school year
      • Documentation of diabetes screening
      • Documentation of lead screening
      • Health history portion of physical examination form must be completed and signed by a parent/guardia
    • Immunizations
      • Diphtheria, pertussis, and tetanus (4 or more doses of DPT with the last dose being a booster and having been received on or after the 4th birthday)
      • Polio (4 or more doses of TOPV/IPV with the last dose being a booster and having been received on or after the 4th birthday)
      • Measles, Mumps, Rubella (MMR) (2 doses of live measles virus vaccine with the first dose at 12 months of age or older and the second dose no less than four weeks (28 days) after the first dose)
      • Varicella (Chicken Pox) (2 doses of varicella vaccine with the first dose at 12 months of age or older and the second dose no less than four weeks (28 days) after the first dose, physician’s statement verifying disease history, or laboratory evidence of varicella immunity)
    • Vision Exam
      • Completed by a licensed optometrist or ophthalmologist
      • Must be completed within the calendar year prior to the start of school
    • Dental Exam
      • Completed by a licensed dentist
      • For the 2020-21 school year, the exam must be performed sometime after November 15, 2019 and before May 15, 2021

     

    GRADE TWO

    • Dental Exam
      • Completed by a licensed dentist
      • For the 2020-21 school year, the exam must be performed sometime after November 15, 2019 and before May 15, 2021

     

    GRADE SIX

    • Physical Exam
      • Dated within one year prior to the first day of the school year
      • Documentation of diabetes screening
      • Documentation of lead screening
      • Health history portion of physical examination form must be completed and signed by a parent/guardian
    • Immunizations
      • Diphtheria, pertussis, and tetanus (3 or more doses of DPT with the last dose being a booster and having been received on or after the 4th birthday)
      • Tetanus, diphtheria, acellular pertussis (Tdap) (one dose regardless of the interval since the last DTaP, DT or Td dose)
      • Polio (3 or more doses of TOPV/IPV with the last dose being a booster and having been received on or after the 4th birthday)
      • Measles, Mumps, Rubella (MMR) (2 doses of live measles virus vaccine with the first dose at 12 months of age or older and the second dose no less than four weeks (28 days) after the first dose)
      • Hepatitis B (3 doses administered at recommended intervals)
      • Varicella (Chicken Pox) (2 doses of varicella vaccine with the first dose at 12 months of age or older and the second dose no less than four weeks (28 days) after the first dose, physician’s statement verifying disease history, or laboratory evidence of varicella immunity)
      • Meningococcal Conjugate (MCV-4) on or after the eleventh birthday.
    • Dental Exam
      • Completed by a licensed dentist
      • For the 2020-21 school year, the exam must be performed sometime after November 15, 2019 and before May 15, 2021
